What do I Need to do for MIPS?

To maximize your MIPS score, you will need to pursue Improvement Activities in addition to Quality Measures and Promoting Interoperability. Smaller practices (fewer than 15 providers) need to perform 1 high or 2 medium weight Improvement Activities. Larger practices need to perform 2 high or 4 medium weight Improvement Activities.

The Referral Portal is a powerful ally to tackle Improvement Activities in your practice and increase your MIPS score. Click on an activity name to find out how Sophrona’s Referral Portal can help.

Improvement Activities

Care coordination agreements that promote improvements in patient tracking across settings

Referral Portal Resources section provides:

  • A custom care coordination agreement template available to referring doctors.
  • Customizable documents that explain the flow of information and establish expectations between the referring doctor and your setting.

Referral Portal provides patients with emailed information that sets their expectations consistently with the care coordination agreement.

Referral Portal tracks the patient through the entire process so that referring doctors know what is happening and what treatment is planned for the patient.

Care transition documentation practice improvements

A patient-centered action plan outlining how to use the Referral Portal to improve patient awareness of status and next steps.

  • Emails sent to patients by Referral Portal can be customized for your practice in order to set patient expectations in a manner consistent with care coordination agreements.

Care transition standard operational improvements

Within the Resources section of Referral Portal, we provide a customizable 30 day patient centered action plan template.

Practice improvements for bilateral exchange of patient information

Complimenting your EHR – Maximizing your Opportunities

If your practice has an EHR in place, you may choose to continue sending transition of care summaries and letters to other doctors via fax or direct through your EHR. Keep in mind, many ODs don’t have their own EHR. Using Referral Portal allows external ODs to exchange information in a manner that ensures you receive MIPS credit for qualified activities.

  • Send a Transition of Care Summary (Promoting Interoperability)
  • Close the Referral Loop (Quality Measure)
  • Use the Referral Portal in parallel to exchange more customized secure messages including images, notes, lab results, answering clarifying questions, etc.

If you’d like to learn more about using the Referral Portal to tackle these Improvement Activities in your practice and increase your MIPS score, Sophrona can help. Contact our team today.
